What is lipoma?

A lipoma is a benign soft tissue tumor composed of adipocytes (very rarely, it is malignant (liposarcoma)) and is a so-called " lump of fat ". It has no adhesion to the skin and is a common and common occurrence. It may be confused with atheroma and atheroma, but it does not purify or give off a smell.

Causes of lipoma

Lipomas are made up of more cells that store fat, but the exact cause is currently unknown. Although ripe adipocytes have no proliferative capacity, even in adulthood, undifferentiated cells that can differentiate into adipocytes remain everywhere around capillaries, becoming a source of adipocytes. Are believed to be present. Tumor growth is assumed to be caused by some abnormality in these undifferentiated cells. However, some chromosomal abnormalities have been found in nearly 80% of lipomas, and it is speculated that the genetic abnormalities induced by these chromosomal abnormalities are involved in adipocyte differentiation and proliferation.

Symptoms and types of lipoma

  • Usually no pain
  • In many cases, you will feel swelling or lumps on your skin and be examined.
  • The size of lipoma is as small as 1 cm in diameter to as large as 10 cm or more in diameter.

The skin has epidermis/dermis/subcutaneous adipose tissue from the top and muscles and bones below it, but most lipomas occur in the layer of subcutaneous fat. On rare occasions, it can occur within the muscle (inside the muscle or between muscles). There is no change in the skin just above the bruise, and it is seen as an elastic lump under the skin.

There is also something called "angiolipoma" that is painful. It has a small diameter of about 1 to 2 cm and is rich in vascular components. When it is removed, it is rich in blood flow and looks a little redder than normal lipomas.

Lipomas grow and grow. It will not disappear unless it is removed by surgery. Therefore, we recommend early treatment/excision surgery.Below, we will look at the characteristics of each type of lipoma.

Clinical symptoms

Diffuse lipomatosis

It is a disease in which lipoma is seen throughout the body, such as the extremities and trunk. It rarely occurs in infants under the age of two. In some cases, lipoma is found not only in the subcutaneous adipose tissue but also in the muscle and internal organs.

Benign contralateral lipomatosis

It is a rare disease in which lipomas are seen symmetrically around the neck, shoulders, upper arms, chest, abdomen, thighs, etc. The tendency is often seen in those who consume a lot of alcohol.

Lipomatous nevus

A tumor consisting of mature adipocytes that develop from adipose tissue in the dermis. Often, the skin surface rises relatively large. It is commonly found on the buttocks, hips, and thighs.

Pathological classification

Angiolipoma

It is a lipoma with many blood vessels in mature adipocytes. Slightly stiff and may be painful to pinch. It can occur frequently throughout the body. It is often small and can be removed with a relatively small incision.

Fibrolipoma

It is a common lipoma. Many collagen fibers are found in mature adipocytes. It tends to form on the back neck, upper back and other areas where pressure is likely to be applied.

Myolipoma

It is relatively deep and often in the muscle. It often occurs in the back of the neck, and the capsule may be unclear. It is a little difficult to remove and may not be completely removed without some incision.

Spinal lipoma

It is one of the latent spina bifida. Spina bifida is a disease in which the bones of the spine cannot be fused and are divided into two during the fetal period. There is a lipoma above the anus and the spinal nerves are pulled toward the buttocks, causing neuropathy. As a result, symptoms such as bladder-rectal disorder (disorders of urination and defecation) and disorders of lower limbs (dyskinesis, sensory disturbance, deformity) may occur.

Polymorphic lipoma

A lipoma that contains various cells in the lipoma. This is a rare symptom.

A case resembling a lipoma

Exostosis

It is one of the benign bone tumors. Bone and cartilage are created in areas where they should not be, and the bone around the joints is protruding. You can do more in your teens. It can be divided into single and multiple. It is often found in the tibia, femur, humerus, and phalanges of the fingers when it is solitary, and can occur anywhere if it is multiple.

Where can lipomas go? Site of onset

Lipomas can occur anywhere on the body. The areas that are easy to make are the neck, shoulders, and back. Next are the thighs, lower legs, and buttocks .

When can lipomas occur? Time of onset

Although it is thought to occur at an early age, it is discovered slowly because it develops slowly. It is rare in people under the age of 20 and is often found in the 40s and 50s.

Who is more likely to have a lipoma? Type that is easy to develop

It is said to be common among women and obese people. Also, there is no initial symptom, and you notice it as a bump when it gradually grows. It usually grows over several years, and if it grows suddenly every few months, then a malignant tumor must be considered.

Lipoma Treatment

Diagnosis of lipoma

At the Cosmetic Surgery Clinic, clinical examinations and medical examinations are combined, and imaging tests are performed as necessary. Image examination includes echo examination, CT examination, MRI examination. Diseases that require distinction include skin-derived tumors and soft tissue sarcomas (malignant tumors). Since it may be difficult to distinguish it from malignant differentiated liposarcoma on the image, it is surgically removed and histopathologically examined. If necessary, we may refer you to a medical institution.

Treatment of lipoma

Radical surgery is used for removal. Surgery with local anesthesia is possible at Cosmetic Surgery Clinic. Most lipomas are benign tumors and do not require emergency surgery, but lipomas grow slowly but gradually and do not disappear or shrink spontaneously. Very rarely, there is a possibility of malignancy such as liposarcoma and malignant soft tissue tumor, so pathological examination is important.

Normal lipoma treatment flow

Lipomas generally require an MRI scan first. Usually, in general hospitals and university hospitals, the MRI examination itself may not be booked for more than a few weeks to several months, and the adipose surgery itself is often hospitalized.
